About the role As a member of the Dementia Respite Cottage, You will be part of the Leadership team and key in the strategic drive and delivery of respite services for clients living with cognitive impairment, BPSD and complex health conditions. The Site Lead (EN) works within the Respite Care Services team to deliver the SCC vision of supporting clients wellbeing through leading the mission of “Doing with, not for” approach to support clients to maintain their quality of life. This role will see you working closely with the Registered Nurse and will support a person-centred care within our dementia specific environment. In addition to this you will monitor and review client’s care needs, identifying early signs of decline or changing needs, escalates to the RN, and working with the Respite team to implement strategies that support the client's ongoing participation and holistic wellbeing. Key Responsibilities include but are not limited to; - Provide nursing services in line with the Scope of Practice of, and Competency
Standards for, Enrolled Nurses (EN) to integrate knowledge and own experiences to devise and achieve agreed client outcomes. - Support the respite assistants to identify strategies that will support clients to
participate in physical, social and mentally engaging activities. - Ongoing monitoring and evaluation of activities, service quality and client outcomes
to ensure it meets client’s wellbeing and overall satisfaction of the service. - Act to resolve immediate care/service delivery problems in a timely manner
- Model and promote an inclusive and positive team culture.
- Work with the Programs Manager - Respite & Social Services to ensure the respite
programs are compliant with Organisational and Home Services procedures.
About you We are looking for people who will be the difference in the lives of our residents and clients, and who are passionate about the translation of evidence based practice to the community respite environment. The successful candidate will have the following skills and experience: - Diploma of Nursing with current unrestricted AHPRA registration as an Enrolled Nurse
- current First Aid Certificate (HLTAID011) & Current CPR Certificate (HLTAID009)
- 5 or more years of dementia care experience
- Extensive knowledge of Dementia, its progression and management of behavioural changes of the disease.
- Ability to lead and support in a team environment
- experience with case management of the holistic needs of clients
- Demonstrated high order verbal and written communication skills and ability to relate with all levels of staff,client, internal and external stakeholders. Significant interpersonal, negotiation and influencing skills that will foster the co-operation and support of others.
- Highly developed organisational and time management skills with an ability to work autonomously and within a team
- experience in the use of community aged care systems and processes such as MyAgedCare, CHSP, ACAT, RAS etc.
- proficient computer and keyboard skills and the ability to use Google Suite (e.g. Docs, Sheets, Gmail etc.) and various in-house databases
- Applicants who hold an additional Certification in Dementia Care will be highly regarded
